All telephone inquiries should be made by calling 715-258-1451.  If it is determined that a formal referral is appropriate, the following steps should be taken:

 

• The child's respective County Department of Social Services should be contacted and be given all pertinent information.

 

• The County Department of Social Services should then send us a referral package (including medical history, case history, psychological evaluation, etc.) with formal letter of intent.

 

• We then review the material and set up a time to meet with the child, his/her family, and referring agency.

 

• We do accept out of state referrals, and/or private pay referrals.
